We prefer a completed EPL application (link) but we can offer a non-binding indication based on the following:

1. Name and address of applicant
2. Number of employees (split out full-time, part-time, seasonal and independent contractors that you wish to cover)
3. Nature of operations
4. Details of any EPL claims or circumstances in the last five years, including any amounts paid or reserved in settlement or defense.
